The following forms should be used by those conducting research at Trevecca. If you have questions, please consult with your advisor or contact the IRB chair at IRB@trevecca.edu.

 

Application Form

Trevecca Nazarene University has partnered with Axiom Education, LLC., to manage all IRB applications through an online program called Mentor. Once you have completed the required IRB training and have earned your certificate (instructions located on our Training page), you may log in to the Mentor program and begin completing your IRB application: https://www.axiommentor.com/login/shibLogin.cfm?i=trevecca

Informed Consent Form

Informed consent is required for most research projects. The researcher should confer with his or her faculty adviser to determine the appropriate consent form to use. A sample of the consent form(s) should be included with the application. The links below provide some sample consent forms, which should be edited so that they are specific to your research project.

For Research Involving Minors – Assent Guidelines

For research involving minors, parental consent AND minor assent are required.  Below are some guidelines pertaining to this process:

In all cases, parental consent must be received in writing (see sample form above).  In addition, minors must provide their assent to participate in the study.  Depending on the participant’s age and developmental level, assent will be either oral or in writing.

  • For participants in 1st grade and below, assent is to be obtained orally.  In order to obtain this assent, researchers will read a written script to potential participants whose parents have already provided consent.  This script, along with the names of those who have assented and those who have not, will then be signed by the researcher and a witness.  See Oral Assent for 1st Grade and Below sample.
  • For participants in 2nd to 8th grade, the minor’s assent is to be obtained in writing along with parental consent.  The script is to be written in language accessible to participants in those grades.  If possible, include the assent and consent on the same form.  See Combined Assent and Consent Form sample.
  • For participants in 9th to 12th grade, the minor’s assent is to be obtained in writing along with parental consent.  The script should be similar in wording to the consent form provided to parents.  If possible, include the assent and consent on the same form.  See Combined Assent and Consent Form sample.

*In addition to grade level, minors’ developmental levels should also be considered when deciding on oral vs. written assent and the specific wording used in the assent statement.

Class Research

Certain classes require all students to conduct research projects involving human subjects under the guidance and supervision of the faculty member teaching the class. For these classes, follow the Mentor link above once you have completed the required IRB training.
