FormsThe following forms should be used by those conducting research at Trevecca. If you have questions, please consult with your advisor or contact the IRB Chair.
Application Form
Click on the link below to download the IRB Application form. Be sure to save it to your computer. Fill out the form electronically and email the completed application form to the Chair of the Institutional Review Board: IRB@trevecca.edu. A print copy of the signature pages should be sent via regular mail, along with the certificate of completion of the training course. If preferred, the application form can be printed and mailed to the IRB Chair. Nashville, TN 37210 The following checklist may be helpful in completing and including all information needed for IRB review. If you have any questions, contact your advisor or the IRB Chair. Informed Consent Form Informed consent is required for most research projects. The researcher should confer with his or her faculty advisor to determine the appropriate consent form to use. A sample of the consent form(s) should be included with the application. It can be emailed to the IRB chair as an electronic attachment, or it can be printed and submitted via regular mail. The links below provide some sample consent forms. Class Research Certain classes require all students to conduct research projects involving human subjects under the guidance and supervision of the faculty member teaching the class. For these classes, the following form can be used to secure IRB approval of the research projects of the class. |
